Life Care Planning

Our Life Care Planning process begins with a detailed interview with the evaluee and a comprehensive review of relevant records. When appropriate, we also conduct in-home evaluations and speak with family members to gain additional insight into the individual’s daily routines and support needs. Throughout the process, we consult with treating physicians and other healthcare providers to document their recommendations and incorporate their medical guidance into our life care plan.

The resulting life care plan provides a clear, evidence-based projection of the individual’s future medical and rehabilitation needs related to their disability. These may include routine medical care, surgical or other invasive procedures, medications, medical supplies, orthopedic equipment, therapeutic services, orthotics, prosthetics, home health care, transportation, home modifications, and other supports designed to promote independence and quality of life.
